Forward Mutual Insurance Company (“Forward”) respects the privacy of our customers, applicants and claimants. We are committed to protecting the privacy of information you entrust to us as described in this Privacy Policy (“Policy”). We do not disclose any non-public personal information about our policyholders or claimants to any third parties except as is permitted by law. Any such disclosures are made for the purpose of transacting the business of your insurance coverage or your claim. We do not sell or market your non-public personal information.

 

This Policy describes the personal information that we collect from you and how that information may be used or disclosed by us. We may make changes to this Privacy Policy from time to time. We will provide notice to you in advance of any changes that may affect your rights under this Privacy Policy. The last time we made a change is the date at the top of this policy.

We collect, retain and use nonpublic personal financial information about you for the purpose of serving your insurance needs and administering your relationship with us. Depending on the kind of relationship you have with us, we will collect an appropriate level of information about you. We may collect information about you from the following sources:

  • Most of the information we receive comes directly from you on applications for insurance, claim and other forms, and when you communicate with us. Examples are your identifying information such as your name, address and date of birth, driver’s license number, social security number and bank account information for policyholders who select direct withdrawal from their bank account for premium payments.
  • Some information comes from third parties such as consumer reporting agencies, insurance agents and brokers, insurance companies and other third parties that you have authorized to provide us with information or as is otherwise permitted by law. Examples are credit reports or claims history from consumer reporting agencies and information that you give to your insurance agent in order to obtain insurance.
  • Information we receive in the process of handling and settling claims with which you may be involved (such as damage reports and medical records). For example, we may engage an adjusting firm to review damage to your property and report to us.
  • We also use publicly available sources of information, such as government data bases and social media.
  • We do not gather any personal information electronically from visitors to our website.

We do not disclose nonpublic personal financial information about our customers or former customers to third parties except as permitted by law.

 

How We Protect Your Nonpublic Personal Financial Information

We restrict access to nonpublic personal financial information about you to those employees who need to know that information to provide the products or services that you have requested. Third parties with whom we share customer information are provided only such information that we believe is appropriate for their specific function and they are bound by the same confidentiality guidelines. We maintain electronic and procedural safeguards that keep your nonpublic personal financial information in our possession secure.
